We aim to bridge the gap between moving from 24 hours residential or foster care to a reduced supportive environment in the community, emphasising on young people making decisions and taking responsibility for their actions through personalisation and social inclusion.
Whilst the young people may have grown out of a children’s home or foster care this does not in any way mean that they are ready to cope with the responsibility of looking after themselves, and dealing with some of the complex issues that they have had to try and deal with in their lives. The aim of the service is to help young people to make the difficult transition from care to living independently in the community.
